If you want to report an aviation-related observation or incident involving UAP,
unidentified aerial phenomena, please take all of the following confidential steps.

1. Use these links to download the appropriate form:

Print and Complete the form and mail to:

2. Contact the FAA Aviation Safety Reporting System and make a confidential report. It
is appropriate to report these incidents to the ASRS to enable safety administrators to
consider these events within the context of aviation safety.

To report an incident or situation to ASRS, download the appropriate Reporting Form
from the FORMS Page on the ASRS website, print, fill out and mail the completed
